An SOP describes one recurring, clearly bounded step of work — not an entire process area. If you need more than seven steps or several decision points, it's probably more than one SOP.

Immediately whenever the process changes, regardless of a fixed schedule. It also helps to spot-check quarterly: is the process still being executed exactly as documented?

The person who actually performs or owns the process day to day — not just a manager. An SOP signed off only at the leadership level often misses the practical edge cases.
